If you should choose to print out forms to bring with you, please select the "Client Information" form.  

If you would like to print the body map to help show the location of your issues, that would be helpful also. 

The remaining forms are for special cases where a physician is needed to either refer you to us or where you have a health risk that would require a physician to give us permission to give you a massage. 

You are, of course, always welcome to fill out a feedback form, or you may wish to leave a comment on or "Testimonials" page by emailing us with your feedback, which will be posted shortly thereafter.
